Efficacy of using bedaquiline in treatment of tuberculosis patients with multiple and extensive drug resistance

The objective: to compare treatment efficacy of pulmonary tuberculosis patients with multiple (MDR) and extensive drug resistance (XDR) using a standard chemotherapy regimen the containing bedaquiline. Subjects Methods. A retrospective study was conducted analyze outcomes in 219 MDR/XDR Mycobacterium (MTB) aged 18-75 years old who were treated in-patient out-patient facilities St. Petersburg from 2016 2019. Treatment analyzed by subgroups formed taking into account patterns addition bedaquiline regimens: Ia – n = 94 (MDR TB; Bq-) IIa ‒ 80 (XDR Bq-); Ib 22 Bq+) IIb 23 Bq+). Clinical laboratory parameters patients, severity comorbidity assessed before start treatment, after 3, 6, 12-14 18-24 months therapy, as well therapy completion during follow-up period. Statistical analysis carried out Statistica 10.0 methods parametric nonparametric statistics SPSS 16.0 software, where p ≤ 0.05 considered significant. Results. XDR TB Subgroup significantly more likely have high level according Charlson index (from 5 6 points) 34.7% cases versus 5.0% IIa. At all stages MDR Bq+ (Ib), sputum conversion recorded often Bq- when examined at stages, maximum effectiveness achieved (90.9%). (IIb, also higher no (60.8% 25.0% (IIa, Bq-), χ2 4.61, < 0.05). Conclusions. Achieved Subgroups Bq provided positive impact on default rates, that low percentage those subgroups. An ineffective course regimens observed 42.5% Ia, while this rate made only 0.09% Ib. highest added regimen.
